This form is required by each Scout for each activity for which transportation is provided by the Troop. This includes, but
may not be limited to, campouts, canoe trips, hikes, lock-ins, and patrol activities. Please fill out the
Activity Consent Form
and give the signed copy to the Tour leader or an Assistant Scoutmaster attending the activity.
Use this form for weekend campouts and day events. You do not need a doctor's signature for this form which is used for activities
less then 72 hours in length. For Summer Camp and High Adventure, you must use the Medical Form Below.
Use this form for Summer Camp & High Adventure or for any activity with a duration over 72 hours. This form does require a doctor to
review and sign off the form using Part C.
For High Adventure, please make sure you bring the Risk Advisory to your doctor for review. Also, please make sure your doctor
checks the Part C Examiner's Certification question about High Adventure and Risk (see below).
Examiner's Certification for High Adventure
This form is to be filled out by the fund raising chair or leader. This form must be approved in advance of the fund raising by the Chartered Organization (Lord of Glory Lutheran Church), the Troop leaders, and then the Council for each fund raiser the Troop would like to conduct. This form allows the various groups to review the purpose of and the type of fund raiser to ensure it matches with the ideals of our Chartered Organization and the Boy Scouts of America.
Use this form to submit your expenses and receipts for reimbursement.
